We are very excited to announce that the Charlestown High School Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.

This will allow:

• Coaches and trainers to have emergency information on each student through their mobile device
• Parents to retrieve their information if an athlete decides to move and transfer

You may review your data at any time to verify it is correct. You will be required to sign your forms once per year and after any update.

We will be sending out the Parent Playbook to get started in FinalForms. If you require any support during the process, scroll to the page bottom and click “Use Support.”

We are asking that ALL parents of student-athletes use FinalForms. The Charlestown High School Athletic Department will no longer be accepting paper copies of the packets. The only form we will accept on paper is the Physical Examination Form.

We will be hosting a parent meeting for all student-athletes, and their parents, on Tuesday, July 25^th at 6:00PM in the CHS Sports Arena to discuss the upcoming season, and to go over some things from FinalForms. A computer lab will be open and available for parents who would like to register their students that night.
